Position: Treatment Adherence Counselor
Job Requirements include:
• Ability to work within the context of a team and take direction;
• Self-starter, good communication (written/verbal) and interpersonal skills a must;
• Strong working knowledge of HIV Treatment Adherence and substance abuse issues;
• Possessing strong follow-up, organizational and documentation skills;
Job Responsibilities include:
- Provide one to one HIV/AIDS, treatment adherence, STD risk reduction and substance abuse harm reduction assessment/counseling.
- Utilizing the cognitive behavioral therapy (CBT) approach
- Furnish referrals for needed services such as primary care, drug treatment, housing, entitlements, etc.;
- Maintain and monitor all documentation of charts for assigned caseload;
- Submit client and program information to Data Input department for each service rendered to clients on a daily basis;
- Provide data and demographics of client services to Director of Case Management & Treatment Adherence on a weekly basis;
- Stay current on HIV/AIDS information, entitlement policies and procedures, and traditional/alternative treatments
- Facilitate Support Groups;
- Assist in recruitment and registration as required;
- Assist in other related duties and assignments as required.
Qualifications:
- Bachelor’s degree or equivalent combination of high school degree and experience in the human services field;
- At least two years experience working with HIV/AIDS, substance abuse and criminal justice issues;
- Spanish Speaking a plus;
- Knowledge of computers, prior database experience.
Salary range: Low-Mid 30’s (commensurate w/experience)
